This form provides important information that can be reviewed prior to your session to maximize our time together. Please click on the age-appropriate form below. The link will direct you to a PDF form that you can fill out online. After completing the questions, please review then save your responses by clicking on the Download/Save icon.
Download Instructions
- Refer to the RED arrow on both illustrations (Firefox and Chrome) to see where to download the file.
- After you have saved your file, you will need to open the file with either Adobe Reader or any PDF Reader. Do NOT use Microsoft Word, it will not open the file correctly.
- Choose where you want to save the form. Example: Desktop or Documents
- Rename your file by putting your last name in front of the file name (i.e. Jones Initial Health Assessment).
- “Save as Type” (located just below file name), select “Adobe PDF” and click on “Save”.
- E-mail the completed form as an attachment to: info@ascension-holistichealth.com. Type the name of the form in the subject line (i.e. Jones Initial Health Assessment).
